This Remediation Guidance summarizes what the staffs of the U.S. Consumer Product Safety Commission (CPSC) and the U.S. Department of Housing and Urban Development (HUD) believe is an effective approach to address potential health and safety issues for the remediation of houses affected by problem drywall, given the information now available. Initial studies found a strong association between the presence of problem drywall and corrosion of metal in homes. Based upon those findings, the CPSC and HUD have developed this guidance that focuses on the replacement of problem drywall and building components for which drywall-induced corrosion might cause a health or safety problem. This version supersedes prior versions of this Guidance.

Attached at Tab A is Sandia National Laboratories’ (Sandia or SNL) final report (Report on Accelerated Corrosion Studies of Electrical Components) on their analysis of corrosion of electrical components exposed to selected contaminant gasses at elevated temperature and humidity to simulate 40 years of exposure to the approximate conditions present in homes with problem drywall. This work was undertaken as part of the U.S. Consumer Product Safety Commission (CPSC) staff’s multitrack plan to assess the health and safety impacts of problem drywall.

Emergency Exit Signs Recalled by Best Lighting Products Due to Failure to Illuminate

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: LHQM LED Exit Signs with Emergency Lights

Units: About 450

Manufacturer: Best Lighting Products, of Pataskala, Ohio

Distributor: Lithonia Lighting, a division of Acuity Brands Lighting Inc. of Conyers, Ga.

Hazard: The fixtures can malfunction and fail to illuminate in the event of a power failure. This could result in a failure to provide adequate lighting to guide building occupants to an exit in the event of an emergency.

Incidents/Injuries: There were ten reports of signs malfunctioning during testing after installation. No injuries were reported.

Description: The recalled exit signs have emergency lights fixed at either end with "EXIT" in red or green in the center. The fixtures are installed in commercial buildings, such as hotels and office buildings. Affected model numbers can be found on a label affixed inside the fixture housing and on the outside of the packaging, and include: LHQM LED G M6, LHQM LED R M6 and LHQM LED R HO M6. Affected models have the following date codes on the inside fixture housing: 1003S10, 1004S10 or 1005S10. Dates on the packaging carton include any calendar dates in March, April or May 2010.

Sold to: Authorized distributors nationwide from January 2011 through May 2011 for $100 to $200.

Manufactured in: China

Remedy: Consumers should contact Lithonia Lighting for a free replacement product.

Honeywell Recalls Electric Baseboard and Fan Heater Thermostats Due to Burn Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Electric Baseboard and Fan Heater Thermostats

Units: About 77,000

Importer: Honeywell International Inc., of Morris Township, N.J.

Hazard: The thermostats can overheat, causing them to melt and smoke. This poses a burn hazard to the consumer.

Incidents/Injuries: Honeywell has received 16 reports of thermostats melting. There have been no reports of injuries.

Description: The recalled thermostats are rectangular, white, programmable thermostats used to control electric baseboard and fan heaters. “Honeywell” or “Cadet” is printed on the front of the thermostats that come in various sizes. The model number and four-digit date code are printed on a label inside the front cover of the thermostat. The model numbers listed below are included in this recall. Only models with date codes beginning with 00, 01, 02, 03, 04, 05 or 06 are included.

Remedy: Consumers should immediately stop using the recalled thermostats by setting the thermostats to 45 degrees or turning them off. Only models with a “B” in the model number have an off switch. Consumers should contact Honeywell for a free replacement installed by Honeywell.

Philips Lighting Recalls EnergySaver and Marathon Compact Fluorescent Dimmable Reflector Flood Lamps Due to Laceration Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: EnergySaver a/k/a/Marathon or Marathon Classic Compact Fluorescent Dimmable Reflector lamps

Units: About 1.86 million

Manufacturer: Philips Lighting Company of Somerset, N.J.

Hazard: The glue that attaches the glass outer envelope or globe to the body of the lamp can fail allowing the glass outer envelope to fall and strike persons and objects below, posing a laceration hazard to consumers.

Incidents/Injuries: Philips has received 700 reports of lamps where the glue failed and the glass outer envelope fell, including two reports of minor injury and three reports of minor property damage.

Description: This recall involves Philips EnergySaver, a/k/a Marathon and Marathon Classic Compact Fluorescent dimmable reflector lamps, models R30, R40 and PAR 38 manufactured between March 2007 and May 2010. The following are the model numbers and Universal Product Code (UPC) numbers of the lamps included in this recall:

Model numbers are printed on the white ceramic area at the base of the lamps. For boxed products, the UPC number is on the bottom of the box. For products in blister cards, the UPC is at the top right corner of the back of the package.

The affected products also have dates codes from March 2007 through May 2010 as follows:

UL Warns of Ground Rods with unauthorized UL Marks

Northbrook, Ill., USA - April 14, 2011: Underwriters Laboratories (UL) is notifying Authorities Having Jurisdiction (AHJs), electrical contractors, electricians and retailers that the ground rods identified below bear unauthorized UL Marks for the U.S. and Canada. These products have not been evaluated by UL to the applicable Standard for Safety for the U.S. and Canada and are not authorized to bear the UL Mark.

UL Warns of LED Lighting Controller with Unauthorized UL Mark

ADP Recalls to Repair Unit Heaters Due to Fire Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: ADP FOA series unit heaters

Units: About 400

Distributor: Advanced Distributor Products (ADP) LLC, of Grenada, Miss.

Manufacturer: Lennox Industries Inc., of Richardson, Texas or Advanced Distributor Products (ADP) LLC, of Grenada, Miss.

Hazard: Some heaters were manufactured without a required flame rollout switch, which is a back-up device that shuts down the heater in the event of a heater failure. This poses a fire hazard.

Incidents/Injuries: None reported.

Description: This recall involves ADP FOA series unit heaters with heating capacity, model and serial numbers listed below. These unit heaters are separated combustion and gas-fired. The brand name "ADP", the model number and the serial number can be found on the nameplate located inside the control cabinet.

AmerTac Recalls Night Lights Due to Fire and Burn Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: LED Night Lights

Units: About 261,000

Importer: American Tack & Hardware Co. Inc. (AmerTac), of Saddle River, N.J.

Hazard: An electrical short circuit in the night light can cause it to overheat and smolder or melt, which can burn consumers or result in fire.

Incidents/Injuries: AmerTac has received 18 reports of the night lights smoking, burning, melting and/or charring, including three reports of minor property damage and one of a minor burn injury.

Description: Three night light models are being recalled: model numbers 71193, 71194 and 327879. Only those with KML molded on the back are being recalled. All three models have KML, ETL, the AmerTac™ logo and the model number molded on the back of the night lights’ plastic housing. Model number 71193 is a square shaped, white plastic unit with a flat translucent square window on the front and a button for changing the screen color. Model 71194 and 327879 resemble a computer mouse with white plastic housing and inset translucent windows on the front and sides.

Sold at: Hardware stores, lighting showrooms and home centers nationwide from March 2009 through January 2011 for about $7.

Manufactured in: China

Remedy: Consumers should stop using the recalled night lights immediately and unplug them from the wall. Contact the firm for instructions on receiving a full refund.

LED Lamps Recalled by Eco-Story Due to Fire Hazard

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: LED Lamps

Units: About 42,000

Manufacturer: Eco-Story, of Portland, Maine

Hazard: When used without a Class II transformer, the lamp can overheat, posing a fire hazard.

Incidents/Injuries: The company has received two reports of overheated lamps. No injuries have been reported.

Description: This recall involves the LED 12-volt lamps with UL number E316865. The UL number is found on a label located at the base of each lamp.

Sold to: Commercial locations, primarily restaurants, from December 2007 through August, 2010 for about between $19 and $45. The lamps were not sold directly to consumers.

Manufactured in: China

Remedy: Commercial locations should stop using this lamp immediately and contact Eco-Story for a free replacement lamp which does not require a Class II transformer. The company has contacted all known commercial locations that purchased this lamp.

Extech Instruments Recalls Digital Clamp and Multimeters due to Electrocution Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Products: Digital clamp meters and multimeters

Units: 5,100 units

Manufacturer: Extech Instruments, of Waltham, Mass.

Hazard: When the battery runs low, the meters can fail to give an accurate voltage reading, resulting in the operator falsely believing the electrical power is off or low. This poses an electrocution hazard.

Incidents/Injuries: Extech received one report of a meter displaying an incorrect voltage reading. No injuries have been reported.

Description: This recall involves Extech digital clamp meters with model numbers EX612, EX613, EX622 and EX623 and multimeters with model numbers EX540, EX542 and EX570. “Extech” and the model number can be found on the front of the product and the serial number on the rear. Serial numbers that begin with "A" are not included in the recall.

Sold at: Industrial and electrical distributors and wholesalers nationwide and at online tool and test equipment retailers from January 2008 to November 2010 for between $150 and $300.

Remedy: Consumers should immediately stop using the recalled digital clamp meters and multimeters and contact Extech for a free replacement meter.

Name of Product: Xantrex Grid Tie Solar Inverters

Units: About 25,000

Manufacturer: Xantrex Technology Inc. - a subsidiary of Schneider Electric, of Livermore, Calif.

Hazard: A component of the inverter can degrade, causing out gassing within the wiring compartment of the inverter. When arcing occurs, gasses could build and force the compartment cover to be blown off. If the cover is blown off with sufficient force it can injure the user or person, or cause damage to property in close proximity to the inverter.

Incidents/Injuries: Schneider Electric has received five reports of wiring compartment covers being blown off. No injuries or property damage have been reported.

Description: The recalled inverter converts solar photovoltaic voltages into utility grid voltages; allowing the owner to feed power into the electrical grid. The recalled units were manufactured between September 2005 and August 2010. These products were sold under the Xantrex, Sunpower, and General Electric brands. The brand name is printed on the front of the unit.

Siemens Recalls Circuit Breakers Due to Fire Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Siemens and Murray Circuit Breakers, Load Centers and Meter Combos

Units: About 2.2 million

Importer: Siemens Industry Inc., of Alpharetta, Ga.

Hazard: The recalled circuit breakers have a spring clip that can break during normal use, leading to a loss of force to maintain a proper electrical connection in the panelboard. This can lead to excessive temperature, arcing or thermal damage at the connection point, and damage to the panelboard’s electrical insulation and can result in a fire, property damage, or personal injury.

Incidents/Injuries: Siemens has received one report of a circuit breaker spring clip that broke during installation. No injuries have been reported.

Description: This recall involves Siemens and Murray 15 through 50 AMP single and double pole circuit breakers, load centers (circuit breakers that come with an electrical panel), and meter combos (contain a load center and a meter socket). “Siemens” or “Murray,” date codes 0610 or 0710 and the catalog number are printed on a label on the side of the circuit breakers. Date codes between June 2010 through August 2010 are stamped on the inside of the metal box of the load centers and meter combos. The catalog number for the load centers and meter combos is printed on a label inside the metal box door and on the packaging.

Miami Breaker Recalls Counterfeit Square D Circuit Breakers Due to Fire Hazard

The following product safety recall was voluntarily conducted by the firm in cooperation with the CPSC. Consumers should stop using the product immediately unless otherwise instructed. It is illegal to resell or attempt to resell a recalled consumer product.

Name of Product: Counterfeit "Square D" Circuit Breakers

Units: About 43,600

Distributor: Miami Breaker Inc. (MBI), of Miami, Fla.

Importer: General Breakers and Panels Inc., of Miami, Fla.

Hazard: The recalled circuit breakers labeled "Square D" or "SQD" have been determined to be counterfeit by Square D and can fail to trip when they are overloaded, posing a fire hazard to consumers.

Incidents/Injuries: None reported.

Description: The counterfeit circuit breakers are marked as Square D in the QO series. The models are QO115 (about 7,800 units), QO120 (about 26,300 units), QO130 (about 3,200 units), QO215 (about 350 units), QO220 (about 720 units), QO230 (about 1,780 units), QO240 (about 1,130 units), QO250 (about 1,150 units) and QO260 (about 1,150 units). Genuine Square D QO breakers have the following: (1) the amp rating written on the handle in white paint on the front of the breaker, (2) the Square D insignia molded onto the breaker side; (3) a yellow chromate clip with half of the top of the clip visible. If your breaker does not match this description, it could be counterfeit.

Sold to: Electrical product distributors and wholesalers nationwide from March 2005 through July 2006. Single pole breakers sold for between $3 and $4, double pole breakers were sold for between $8 and $9.

Remedy: Consumers should immediately contact Miami Breaker to determine if their circuit breaker is counterfeit and if necessary, arrange a free inspection and replacement.

Recall to Repair: Whitco Company LP Stadium Light Poles Can Fall Over, Posing Risk of Serious Injury and Death

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, today announced a recall of the following consumer product. Consumers should immediately stop using recalled products until they are inspected and repaired.

Name of product: Whitco Company LP poles 70 feet tall or higher

Units: More than 2,500

Manufacturer: Whitco Company LP, of Fort Worth, Texas (which is out of business)

Hazard: The poles can fracture or crack and fall over, posing a risk of serious injury or death to patrons and bystanders from being hit or crushed. The poles range from 1 to 4 tons increasing the risk of death if the pole falls toward a crowded stadium or onto a building.

Incidents/Injuries: As of June 2010, CPSC has confirmed eleven incidents in which Whitco Co. LP poles fell. In one incident, a pole fell through the roof of a school gymnasium causing significant property damage. In two other incidents, the poles fell onto outdoor bleachers causing significant property damage. The latest incident occurred at Integrity Park Inc. in Argyle, Texas. The majority of incidents occurred in Texas with others in several other states where Whitco Co. LP poles have been installed. To date, CPSC is not aware of any injuries. However, some incidents have been close calls, with people exiting the area just minutes before a pole fell. CPSC has also identified more than 50 Whitco Co. LP poles that have not yet fallen, but inspections revealed fractures and/or cracks next to the weld that joins the pole to its base plate. The fallen and cracked poles were located near or around school football stadiums and gymnasiums.

Description: This recall to inspect and repair involves Whitco Co. LP outdoor steel stadium light poles from about 70 to 135 feet and weigh from about 1 to 4 tons. The poles are constructed of steel with a galvanized coating and were manufactured between 2000-2005.The poles can be found at facilities such as recreational parks and fields, schools and outdoor stadiums. Attached is a listing (pdf) of locations that may have the affected poles.

Note: This release does not affect Whitco Pole products.

Sold by: Lighting contractors nationwide from 2000 through 2005.

Remedy: To reduce the risk of injury, Whitco Co. LP light poles should be inspected by an engineer or a Level II non-destructive testing technician immediately to identify cracking at or near the weld connecting the pole to the base plate flange. A visual examination with the naked eye or with a magnifier will not determine the extent of any cracking. Non-destructive techniques such as magnetic particle inspection, dye penetrant, or ultrasonic inspection techniques are recommended. If any cracking or fracturing is found, immediately have the affected poles repaired or replaced by a qualified professional. A design analysis to assess the stresses placed on the pole is also recommended. CPSC recommends that all outdoor steel stadium light poles be routinely inspected by a professional. As the manufacturer has become bankrupt, individual owners of the poles must arrange for inspection.

Ideal Arc-Fault Testers

IDEAL has been receiving reports that the latest Square D CAFCI Breakers (QO/Homeline) breakers coming onto the market are not recognizing the arcing signal from Ideal's arc-fault testers. IDEAL engineers have obtained these breakers and have confirmed the condition in their test lab. IDEAL and Square-D will be working together to correct this issue. In the meantime, the 61-059 and 61-165 products are on hold.

Subject: AFCI Testing

Dear Electrical Professionals,

We have received notification from locations around the country that the latest Square D Combination AFCI breakers (QO and Homeline) coming into the market with 2010 date codes do not trip on the arcing signal of our SureTest® AFCI testers.

IDEAL has tested samples of these breakers in our lab and confirm that we are unable to trip them with our existing product without further product modifications.

We are in contact with Square D in order to learn what has changed with their recent production. The Square D Product Manager indicates willingness to work with us to solve the immediate problem and also come up with a plan to prevent this situation from recurring.

IDEAL is committed to providing Inspectors and Electrical Service Contractors the tools and testers to verify that devices are installed properly and wiring meets minimum standards for performance and safety on every branch circuit.

In the meantime, in consideration of this circumstance, we are placing our existing inventory On Hold until we learn why the UL 1436 arc does not trip these particular breakers.

UL warns of ground rods with inadequate copper plating

Northbrook, Ill., USA - June 10, 2010 - Underwriters Laboratories (UL) is notifying AHJs, electrical contractors, electricians, retailers and distributors that the ground rod, identified below, may not be provided with adequate copper plating. These products display a UL Listing Mark for the U.S.; however, products were found not to comply with the UL safety requirements and are not authorized to bear the UL Mark.

UL Warns of Electrical Metallic Tubing (EMT) With Inadequate Corrosion Protection

 

NORTHBROOK, Ill., May 13 /PRNewswire/ -- Underwriters Laboratories (UL) is notifying AHJs, electrical contractors, electricians and retailers that the Electrical Metallic Tubing (EMT) products, manufactured by Guangdong Walsall Steel Pipe Industrial Co., Ltd. and identified below, may not be adequately protected against corrosion. These products display a UL Listing Mark for the U.S. and Canada; however, products purchased at retailers and distributors were found not to comply with United States or Canadian safety standards and are not authorized to bear the UL Mark.

UL Warns of Underwater Lighting Fixtures With Unauthorized UL Marks

 

NORTHBROOK, Ill., March 12 /PRNewswire/ -- Underwriters Laboratories (UL) is notifying consumers that the underwater lighting fixtures, identified below, bear unauthorized UL Listing Marks. The fixtures are marked with the UL Mark and "E41117," however they were not produced by the UL authorized manufacturer, JULONG POOL & SPA PRODUCTS. These fixtures have not been evaluated by UL to the appropriate standards for safety and are not authorized to bear the UL Mark. It is unknown if these fixtures comply with safety requirements.

Similar underwater lighting fixtures are authorized to bear UL Marks. The differences between the unauthorized and authorized fixtures are provided below.

Outdoor Lighting Fixtures Recalled by American Electric Lighting Due to Shock Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Outdoor Lighting Fixtures

Units: About 900

Manufacturer/Distributor: American Electric Lighting, a division of Acuity Brands Lighting Inc., of Conyers, Ga.

Hazard: Improper wiring in the light fixtures poses a shock hazard to consumers.

Incidents/Injuries: None reported.

Description: This recall involves model AVL outdoor lighting fixtures sold under the American Electric Lighting brand name. The light fixtures are intended for use in parking lots, roadways, commercial environments and office communities. “AVL” is printed on the reflector of each light fixture.

Sold by: Electrical distributors and electrical sales representatives nationwide from September 2008 through September 2009 for between $250 and $800.

Manufactured in: Mexico

Remedy: Consumers should immediately disconnect power to the fixtures and contact American Electric Lighting to schedule a free inspection and repair.

Noncontact Electrical Tester Recalled by Fluke Due to Shock or Burn Hazard

WASHINGTON, D.C. - The U.S. Consumer Product Safety Commission, in cooperation with the firm named below, today announced a voluntary recall of the following consumer product. Consumers should stop using recalled products immediately unless otherwise instructed.

Name of Product: Fluke VoltAlert® Voltage Detector

Units: About 33,000

Manufacturer: Fluke Corporation, of Everett, Wash.

Hazard: The testers can fail to give an indication of live voltage, resulting in the operator falsely believing the electrical power is off, posing a risk of serious injury or death from electrical shock or thermal burns.

Incidents/Injuries: No injuries have been reported.

Description: The Fluke voltage testers look like a pen with a yellow, white and gray body. The testers measure 90 to 1000 volts alternating current (VAC). “Fluke” and the model number are printed on the front of each unit. The recall involves Fluke 1AC-A1-I VoltAlert® tester with the following model and item numbers:
